Fee Waiver Policy

First Course Fee Waiver Policy

The provider offers services without regard to a consumer client’s ability to pay. Consumer clients may be eligible for a reduced fee or fee waiver upon written verification of their household size and income. A reduced fee or fee waiver will be assessed based on 150% of the Federal Poverty Level Guidelines last published by the United States Department of Health and Human Services ( DHHS ) for the household size and financial circumstances. The provider will evaluate all client household sizes and household incomes to determine their eligibility for a reduced fee or fee waiver. Consumer clients do not have to specifically request a waiver.

In order for the provider to determine your eligibility for reduced fees or fee waivers, please submit:

1) household size (if you are determined to be eligible for a reduced fee or fee waiver, you will be required to provide supporting documentation such as income tax returns or another government document that serves as proof of your household size);

2) household income (if you are determined to be eligible for a reduced fee or fee waiver, you will be required to provide supporting documentation such as income tax returns or W2′s, employer letter, 3 most recent pay stubs, unemployment certificate, a statement of benefits obtained from the government or Social Security Administration or leave and earnings statement from the military).

Please Note:

Do not include non-cash governmental assistance (such as food stamps or housing subsidies) when calculating monthly gross income.

Include the income of your spouse to calculate monthly gross income whether or not a joint petition is filed, unless you are separated from your spouse and a joint petition is not filed.

Please forward your household size (along with supporting documentation if you have been determined eligible for a reduced fee or fee waiver), and your household income (along with supporting documentation if you have been determined eligible for a reduced fee or fee waiver) to:
